A document called the Medical Orders for Life-Sustaining Treatment form--often referred to as the MOLST form--has been revised. It records a patient's preferences for life sustaining treatments such as: CPR, ventilation and other interventions. The new MOLST form along with supporting checklists, glossary and instructions are available on the state Department of Health webpage.
